Comprehensive Assessment

In this process, the client’s individual needs are evaluated and a written plan of care is developed. The Comprehensive Assessment includes:

  • Medical and Social History
  • Medication Review
  • Functional Assessment
  • Safety Considerations
  • Financial Analysis
  • Benefit Eligibility Assessment
  • Support System Assessment

During the assessment process, clients will receive a resource notebook for educational purposes. Assessment can be done at our office location or off-site if preferred.

Elder Advocates Incorporated was founded in 1998.  Patrice Antony, owner and founder of the company, is a Physical Therapist and board certified Geriatric Specialist who has worked in the Central FL area since 1981.  She decided to open this practice in answer to the needs of so many of her rehab clients and their families searching for help navigating the medical maze.  The mission of Elder Advocates Inc. is to advise and support seniors, disabled loved ones, and their families in obtaining care services and needed benefits to pay for those services.  The company supports families in many roles including care management, benefit filing assistance, placement needs, and estate settlement to name just a few.  Customer service and support is paramount.  You are not alone!
